Important Online Information Update Directions

 

 

Welcome Back Lakeville Wildcats!

 

Please see the directions below to update your child’s information.  We ask that this process is completed by August 20, 2021.  It is important that we have the updated information on file when your child attends school on the first day which is Wednesday, August 25, 2021.

Directions for Completing the Information Update:

1.  Go to the website address: https://updateregistration.oxfordschools.org/ or access the “Online Information Update” link on our school’s website.

2. Login using the following unique username and password:

3.  Once logged in the system, the program will take you through the registration update process. Please make sure emails are correct. These emails are used by the School Messenger System to send information home from           Oxford Schools.

4.  You will be prompted to complete the necessary information about parents/guardians, free and reduced lunch, student information, medical conditions, concussion awareness, emergency contacts, elementary school closings,                    Acceptable Use Policy (AUP), pesticide notification, student insurance, and a parent notification agreement.

5.  The system will notify you when all forms have been completed.  Thank you for taking time to do this.

 

Additional Information

Below you can find some additional information about the forms you are asked to complete.

 

Free and Reduced Lunch – Under this heading, the link to print off the forms for free and reduced lunch are found. These forms will also be sent home with classroom placements prior to school starting.  The forms must be turned into your child’s teacher or to the kitchen manager or main office to determine if your child will qualify to receive either free or reduced lunch and/or breakfast. 

 

Medical Conditions and Problems – Under this heading you will use the links at the bottom of the page to retrieve the forms necessary if your child will need to take either prescribed medication or over the counter (OTC) medication at the school.  For prescribed medication, you will print off the form and take to your physician to fill out the needed information and to sign.  This form is to be turned in WITH THE MEDICATION to the main office.  Note: Each medication is to have a form filled out and turned into the school.  The link to the form for over-the-counter medicine is on this page as well.  You will print these forms off and complete the needed information and be sure to sign the form!  The form is to be turned in to the main office WITH THE MEDICATION.  No Medication will be administered at Lakeville Elementary without the proper documentation required according to State Law.

 

Under this heading, you may print off a Medical Action Plan (MAP) for the following health conditions: diabetes, severe allergies, asthma, seizure, or a general health condition that is severe enough to interfere with a life function. 

 

Concussion – Parents will find information from the State of Michigan on the health concerns involving concussions. 

 

Emergency Contact and Social Restriction – Parents are to list individuals who may check out their child from school.  If an individual is listed here, the office will not contact the parent first to receive permission.  Also, if there is a court order preventing an individual from having contact or removing a child from school, you are able to put this information here.  The office will need a copy of current court paperwork to enforce this restriction.

 

Insurance – This form is optional for parents.  It allows parents to purchase health insurance for their child by clicking on the link.  The district has no affiliation with the insurance company and merely provides the opportunity for parents to pursue this option if they wish.  Please read carefully.
